Synergy Personal Retirement Savings Accounts (PRSA) and/or PRSA Additional Voluntary Contributions (AVC)

These are the items that are missed most often – we've listed them according to the relevant page on the application form:
- From page 3 – contribution type.You have two choices: PRSA contributions only, including transfer payments not representing AVCs, or Additional Voluntary Contributions only, including transfer payments representing AVCs
- From page 5 – salary deduction.You only need to complete this section if your client wants to make a net pay arrangement
- From page 8 – your signed and dated adviser's declaration. Please also tell us whether you want bonus commission, even if only regular contributions are being paid now. Remember, you'll need to choose your PRSA commission terms at the outset, and they can’t be changed – they'll apply to the whole policy

We'll need to see:
- Proof of your client's date of birth, like a certified copy of their current passport or driving licence
- Proof of their PPSN, like a copy of their P60
- For PRSA AVCs, we'll also need a copy of a recent annual statement from your client's occupational pension scheme
- For transfer payments, a Certificate of Comparison (unless exempt)
- If a transfer payment is subject to or as a result of family law proceedings like a Pension Adjustment Order, please enclose a copy of the Notice or Order
