These are the items that are missed most often – we've listed them according to the relevant page on the application form:
- From page 1 – Personal Public Service Number (PPSN). Please remember to provide your client's PPSN with their annuity application
- From page 2 – Annuity details for all parties. If your client would like their spouse or civil partner to be included in the annuity, we'll need their details too. These include the spouse or partner's PPSN and date of birth as well as the percentage of the main annuity to be allocated to them
- From page 3 – Payment and tax information. Please make sure you've provided an IBAN and bank account name for your client, and advised them to contact Revenue for a tax certificate covering their annuity payments. This makes sure they won't be subject to emergency tax. Their certificate should quote Standard Life's annuity employer reference: 0064349F
Please also remember to tell us what commission you want and provide the agency code we need to pay it to.

As well as a cheque or EFT from the pension provider or QFM, you'll need to provide us with proof of your client’s identity, address and sources of funds and wealth. That's in line with your AML/CFT procedures. These are stated in the financial adviser's declaration you provide with the application:
"I have completed all checks to verify the identity, address, source of funds, and source of wealth of your client to the standards set out in the anti-money laundering and counter financing of terrorism regulations".
You can send verification documentation to us at any time – it won't hold up the processing.
